Bone Diseases 5 Flashcards
Altered condition of bone Normal bone replaced Proliferation of fibrous connective tissue New mineralized component
BENIGN FIBRO-OSSEOUS LESION
Monostotic: Common Craniofacial Polyostotic: Rare McCune-Albright Syndrome: Severe - Cafe Au Lait Macules Bone, skin and endocrine lesions Jaffe-Lichtenstein Syndrome: Mild Bone and skin lesions only
Fibrous dysplasia
What 2 syndromes are associated with fibrous dysplasia?
McCune Albright
Jaffe Lichtenstein

Fibrous dysplasia
Postzygotic GNAS 1 gene mutation – codes for a G-protein and results in hyperfunction of cells in the involved
tissues
Clinical severity: depends on the point in time the mutation occurs.
Early embryonic life in undifferentiated stem cell
Osteoblasts - Multiple bone lesions
Melanocytes - Cutaneous pigmentation
Endocrine cells - Endocrine disturbances
Later embryonic life in skeletal progenitor cell
Osteoblasts - Multiple bone lesions, cutaneous pigmentation
Postnatal life
Osteoblasts - Single bone lesion
Fibrous dysplasia
Benign fibro-osseous process
Abnormal reaction of bone
The most common benign fibro-osseous lesion of the jaws
Probably arises from elements of the periodontal ligament
CEMENTO-OSSEOUS DYSPLASIA
A benign fibro-osseous lesion located in the periapical region of anterior mandible Females (10:1) Blacks 70% Ages 30 to 50 years Asymptomatic, associated with vital teeth Solitary or multiple Early lesions lucent
PERIAPIACAL CEMENTO-OSSEOUS
DYSPLASIA
Middle-aged adults, strong racial predominance for
African-Americans, strong female gender predominance
Multifocal involvement, often bilateral and symmetric, four
quadrant involvement not unusual
Lucent – mixed – opaque, with thin peripheral radiolucent
rim
Usually asymptomatic, possibly dull pain, expansion rare
Associated with simple bone cysts
FLORID CEMENTO-OSSEOUS
DYSPLASIA
Middle-aged adults (Mean = 38 y) Strong female gender predominance Possible racial predominance Posterior mandible commonly Frequently in extraction site Thin peripheral radiolucent rim
FOCAL CEMENTO-OSSEOUS
DYSPLASIA
Benign neoplasm
Age 20 to 40 years
Female predilection
Molar and pre-molar region of mandible, most frequently
Radiolucent to mixed
Expansion, may displace teeth
May arise from elements of the periodontal ligament
Cemento-ossifying fibroma
How will the cemento-osseous dysplasia lesion come out in a biopsy? one piece or many pieces
Many pieces
How will the cemento-ossifying fibroma lesion come out in a biopsy? one piece or many pieces
Comes out in 1 piece
Does ossifying fibroma or fibrous dysplasia have expansion?
Ossifying fibroma
