Definition of ONJ
Exposed necrotic bone that doesn’t heal after eight weeks in the absence of a history of radiation

Poorly fitting dentures
Invasive dental procedures.
Pre-existing dental disease

0-non-specific clinical findings, no exposed bone
1-exposed bone, no symptoms or infection
2-exposed bone with infection
3-exposed bone with infection and at least one of:
Exposed and necrotic bone causing fracture.
Fistula or communication outside oral cavity.
Osteolysis beyond mandible or sinus floor
