What produces BNP?
Myocardial cells when under stress or stretched
Brain
How is BNP produced? (recognise)
Prodcued in myocyte as pro-BNP, converted to pre-pro BNP in the cell
Name of BNP fragement measured in the circulation?
N terminal pro-BNP (inactive)
Can also measure the BNP itself
Actions of BNP (3)
Vasodilation
Increased urinary sodium excretion
Myocyte growth/fibrosis
(tries to reduce strain on heart)
Clearance of BNP (2)
Breakdown by neural endopeptidase
Clearance by receptors
Uses of BNP monitoring?
Diagnosis of heart failure used to rule it out)
Screening for LV dysfunction
Risk stratification in heart failure and ACS
What other conditions can BNP be used to assess prognosis? (recognise) (5)
