What is the first thing you do when you enter a scene.
Verify the scene is safe for both you and the victim
After you verify the scene whta do you assess for?
If the victim is unreposonsive.
If the victim of unresponsive what is the next thing you should do?
AA
Activate EMS via mobile device (if appropriate) and get AED and emergency equipment (or send someone to do so)
After you activate EMS and send someone for emergency equipment/AED what shoudl you do?
Look for no breathing or only gasping and check pulse.
If the victim has no normal breathing but has a pulse you should…
Provide rescue breathing.
At what rate to do you provide rescue breathing to an adult?
1 breath every 5 to 6 seconds or 10-12 breaths/min.
After you provide rescue breathing for 2 min. what should you do?
ARC
Activate EMS (if not already done), continue rescue breathing; check pulse about every 2 minutes.
If you are performing rescue breathing and the victim suddenly has no pulse what do you do?
- Immediately begin 30:2 high-quality CPR-
- Use AED as soon as it is available.
When the AED arrives and your victim still is not breathing and has no pulse what do you do?
Apply pads - check rhythm to determine if it is a shockable rhythm.
After the victim receives a shock from the AED unit what do you do?
Resume CPR immediately beginning with chest compressions for about 2 minutes until prompted by the AED to allow rhythm check). Continue this cycle until ALS providers take over or victim starts to move.
If the rhythm is not shockable?
Resume CPR immediately for about 2 minutes (until prompted by AED to allow rhythm check). Continue until ALS providers take over or victim starts to move.
What are agonal gasps?
Presents as drawing air in very quickly, mouth may be open and the jaw, head or neck may move with gasps. Can be forceful or weak.
How long should you assess for normal breathing?
No more than 10 seconds.
What is agonal breathing a sign of?
Cardiac Arrest
Where do you palpate for a pulse?
Carotid Artery
When do you begin high quality-CPR?
If you DEFINITELY do not feel a pulse in 10 seconds
How long do you palpate for a pulse?
At least 5 seconds but no longer than 10 seconds.
What do you do if opioid use is supsected?
Consider naloxone if available and follow your local protocols.
What is the best way to ensure appropriate hand placement for compressions?
Remove or move clothing
What are high-quality chest compressions?
- Compression-to-ventiliation ratio of 30:2
- Compress the chest at least 2 inches (5cm)
- allow the chest to recoil completely after compression
- Minimize interruptions
Should you move the victim during compressions?
Not unless the victim is in a dangerous envrionment or
if you believe you cannot perform CPR effectively in the victim’s present position or location.
If the victim is on a soft surface such as a mattress what should you do?
Move them to the floor or use a backboard for a firm surface to allow compression of the chest and heart to create blood flow.
What steps do you follow for chest compressions?
- at the victim’s side
- face up on a firm flat surface.
- Position hands
- middle of the chest
- lower half of the sternum
- one hand on the chest and the other hand on top of the first hand.
- Straighten your arms and
- position your shoulders directly over your hands.
- rate 100-120 bpm
- Press down at least 2 inches
- pushing straight down on the sternum.
- Allow for recoil.
- Minimize interruptionos.
What method of opening the airway do you use if you suspect a neck or back injury or trauma?
Jaw thrust
What is the recommended way to open the airway when there are multiple rescuers?
One rescuer can perform a jaw thrust
while another rescuer providesbreaths with a bag-mask device.
The third rescuer will give chest compressions.
What two things should you avoid doing when performing the head tilt chin lift?
- Do not close the mouth completely
- Do not press deeply into the soft tissue udner the chin because this might block the airway.
When should you replace a face shields with a pocket mask?
At the first opportunity.
Steps in using a pocket mask
- at victim’s side
- Place on victim’s face, using the bridge of the nose as a guide for correct position.
- Create a seal
- Use your index finger and thumb (on your arm at top of head) to secure shield.
- Place the thumb of your other hand along bottom edge of the mask.
- Place the remaining fingers of your second hand along the bony margin of the jaw and lift the jaw. Perform a head tilt-chin lift to open the airway.
- While you lift the jaw, press firmly and completely around the outside edge of the mask to seal the pocket mask against the face.
- Deliver each breath over 1 second, enough to make the victim’s chest rise.
When performing multi rescuer CPR how often AND when should you switch who is performing compressions.
- Every 2 minutes or 5 cycles
- While AED is analyzing
- When fatigued
