Toxicity: N + V + D, excessive thirst, drowsy, blurred vision, convulsions
Mechanism of Action: Lithium
Promotes GABAergic neurotransmission
Interactions: Lithium
Avoid NSAIDs Diuretics, ACEIs Antidepressants Carbamazepine Haloperidol
Indications and Forms of Valproate:
Acute mania, prophylactic agent
Sodium, valproaic acid, semi-sodium
Adverse effects: Valproate
Weight gain, GI, blood disorders, fatigue, n, sedation, tetatogenic
Mechanism of action: Valproate
Block NA+ channels, inhibit GABA transaminase, suppress glutamate action, blocks Ca++ channels
Interactions: Valproate
Highly protein bound
Liver metabolised
Indication: Lamotrigine
Depressive episodes
Monitor
Adverse Effects: Lamotrigine
Skin rashes – may need to be withdrawn
Headache, dizziness, N, drowsiness, insomnia
Mechanism of Action: Lamotrigine
Block Na+ channels, inhibit excitatory amino acid release (glutamate and aspartate)
Interactions: Lamotrigine
Plasma conc reduced by: phenytoin, carbamazepine, oestrogens, progestogens, valproate, primidone
Indications: SGAs
Acute mania
Usually olanzapine or quetiapine
