Stimulants -
- Methylphenidate

Adverse effects of stimulants
Methylphenidate - Insomnia, GI effects, CV effects
Avoid abrupt withdrawal
Lisdexamfetamine and Dexamfetamine - Drowsy (driving)
Mechanism of action of Methylphenidate
Increases intrasynaptic dopamine + noradrenaline in frontal cortex

Also – block vesicular MA transporter to increase levels
What interactions should we be aware of with Methylphenidate?
Monitor pulse, BP, weight, height
Anticoagulants, MAOIs, AEDs, antidepressants
What interactions should we be aware of with Lisdexamfetamine and dexamfetamine
MAOIs, antidepressants, moclobemide, rasagiline
Second line ADHD treatment
Atomoxetine
When should Atomoxetine be taken?
Take second dose at teatime so does not disturb sleep
Atomoxetine adverse effects
GI effects, increased hr + bp, prolongs QT interval
Mechanism of action for atomoxetine
SNRI, no CNS stimulant activity
What interactions should we be aware of with atomoxetine?
MAOIs, drugs that prolong QT interval, terbinafine
Name four drugs that could be used as a third line option
Bupropion
Modafinil
Noradrenergic antidepressants
Clonidine

GI, dizzy, dry mouth, fever, headaches, drowsiness

Nausea, vomiting, antimuscarinic, headache
