What is Barretts Oesophagus?
Metaplasia of the oesophageal epithelial lining, whereby the normal stratified squamous epithelium is replaced by simple columnar epithelium
What is metaplasia?
The abnormal reversible change of one cell type to another
What usually causes Barretts Oesophagus?
GORD due to the damage from the reflux of acid causing metaplastic transformation
How is Barretts oesophagus diagnosed?
Biopsy
What are the risk factors for Barretts oesophagus?
caucasian, male, over 50, smoker, obesity, hiatus hernia, family history
What are the clinical features of barretts oesophagus?
history of chronic GORD so retrosternal chest pain, excessive belching, odynophagia, chronic cough and hoarseness
What investigations would you do for Barretts Oesophagus?
OGD and biopsy, will look red and velvety and some pale squamous islands
What is the management for barretts oesophagus?
PPIs, Lifestyle factors
What can Barretts Oesophagus lead to?
Adenocarcinoma
What should be done to check for adenocarcinoma in people with Barretts oesophagus?
regular endoscopy
What is the prognosis for Barretts Oesophagus?
if high grade has a high risk of becoming cancer so should be resected by endoscopic mucosal resection
