Pentobarbital (Neumbutal Sodium)
Barbituate. Contraindicated: any porphyria and pulmonary insufficiency
Barbituates (whole class)
Act by binding to modulatory site on GABAa to prolong actions of GABA (increase Cl- influx).
Marked CNS depression such that they produce hypnosis and can produce anesthesia. Drugs of euphoria- drugs of abuse
Low margin of safety. At high doses: decrease resp. drive
Supra-additive CNS depressive effects by combining 2 drugs esp. w/alcohol. Impairs REM.
Lipid soluble, enter CNS easily (degree of lipid solubility determines onset and duration of action)
Metabolized by liver, and induces hepatic enzymes with chronic use
Tx toxicity: supportive, dont use stimulants, cleared w/alkalinization of urine.
Withdrawal is life threatening.
Absolute contraindication is any porphyria, also pulmonary insufficiency.
