Empiric Tx: Vanc or Dapto (D in pts w/ septic pulmonary emboli)
- MSSA SAB: Nafcillin, Oxacillin, Cefazolin IV

Blood cultures and Echocardiography (TTE, TEE)

SAB Tx Duration
A
- Uncomplicated: 14 days
- Complicated: 4 weeks
- Complicated w/ Metastatic infxn: 6-8 weeks
IV Tx Duration starts from FIRST NEGATIVE blood culture
NEVER PO Tx

S. Pyogenes & S. Agalactiae Bacteremia
A
- Penicillin IV
- then, high dose Amoxicillin PO

S. Pneumoniae Bacteremia
A
- Ceftriaxone or Penicillin
14 days (IV, then PO)

E. Faecalis Bacteremia
A
- Ampicillin IV
- If Amp-Resistant or B-Lactam Ax: Vanc or Dapto

A
- VanA or VanB (-): Vanc
- VanA or VanB (+): Dapto or Linezolid
