Describe secretory diarrhea
Due to net fluid secretion into the intestine. Fluid is isotonic. Diarrhea persists during fasting.
Describe osmotic diarrhea
Solutes within the intestinal lumen cause osmotic water loss.
Subsides when fasting.
Describe exudative diarrhea
Purulent or bloody stools caused by infectious enterocolitis.
Persists when fasting.
Describe Malabsorption diarrhea
Bulky, oily, steatorrhea
subsides with fasting
Describe diarrhea of deranged motility
Really variable, diagnosed by exclusion.
Major causes of infectious enterocolitis
- Enteroviruses
- Adenoviruses
- Rotaviruses
- Caliciviruses
- The various E Colis
- Shigella
- Campylobacter
- Vibrio
- Salmonella
- Entamoeba histiolytica
- Giardia lamblia
- Whipworm, trichuris trichuria

What are the major differences between viral and bacterial enteritidis?
Viral:
- Infects and destroys the epithelium, impedes absorptive function.
- Cause secretory and osmotic diarrhea.
Bacterial:
- can be toxin mediated: C. perfringens, S. aureus,
- Luminal infection: Vibrio, ETEC
- Enteroinvasive infections: Shigella, EIEC
- Bacterial enteritidis is usually more severe than viral.
