Atrial fibrillation definition
Irregular chaotic atrial activity- no P waves
Irregular electric activity of isoelectric line
Variable R-R
Normal narrow QRS
450-600 bpm
Paroxysmal AF
Self terminating usually within 48 hours
Persistent AF
> 7 days
Longstanding AF
> 1 year
Permanent AF
Attempts to convert to NSR have been abandoned
Atrial flutter definition
Regular atrial activity around 300 bpm
Ventricles conduct half the time 2:1 block (150 bpm)
Negative flutter waves
II, III, aVF
Positive flutter waves
V1
Normal PR
120-200 ms
Causes of short PR
WPW: broad QRS and delta wave
Lown-Ganong-Levire: normal QRS
First degree AV block
Prolongued Pr- consistent PR duration
can progress to other forms
Causes of first degree AV block
Electrolyte disturbance
Rheumatic endocarditis
Digoxin BB CCB
Enhances vagal tone
Normal P wave
<120 ms
<2.5 mm
P wave >2.5 mm
P Pulmonale- RA enlargement
P wave > 120 ms
P mitrale- LA enlarges
V1 may have negative component
