Assessment Flashcards
What is included in the Primary Assessment?
- Level of Consciousness
- Breathing Status
- Circulatory Status
If life-threatening condition is apparent. These must be treated immediately.
Carotid pulse present with no radial pulse
Lay the patient flat and elevate his or her feet 8-12 inches.
No Pulse
Begin CPR
Shock
Lay patient flat, elevate his or her feet 8-12 inches, and administer oxygen at 15 L/min
Bleeding
Apply pressure to control any bleeding
Abnormal Breathing
Provide oxygen with a nonrebreather mask. Use a BVM if unresponsive
Unresponsive
Ensure patent airway
Secondary Assessment include;
- Vital Signs
- Further Questions
- Physical Examination
- Treat any life-threatening conditions immediately
When is a reassessment performed?
After the Secondary Assessment at least every 5 minutes for unstable patients and 15 minutes for stable patients.

B/P: Systolic 80 - 110
School Aged (6-12yo) Vital Signs
H/R: 70 - 110
R: 20 - 30
B/P: 80 - 120 / 60 - 80
Adolescent (13-18yo) Vital Signs
H/R: 55 - 100
R: 12 - 20
B/P: 110 - 131 / 64 - 84
Early Adult (19-40yo) Vital Signs Middle Adult (41-60yo) Late Adult (61+yo)
H/R: 600 - 100
R: 12 - 20
B/P: 120 / 80
