Proteins leaking into vessel wall, produces vasc thickening, pink hyaline on micro. Due to benign HTN (inc bp forces proteins out) and Diab (leaky walls). Can progress to CRF

Onion skinning due to thickened sm Musc. Malignant HTN cause. May lead to fibrinoid necrosis of vessel wall, causes AR with fleabite appearance.
