Arsenic poisoning Flashcards
Which form is more toxic, trivalent or pentavalent?
trivalent
What are symptoms of acute toxicity?
intense GI distress, projectile vomiting, diarrhea, hypotension, RENAL TUBULAR NECROSIS
What are symptoms of chronic arsenic toxicity
The THREE D’s: Dementia, dermatitis with altered pigmentation and keratoses, Diarrhea, polyneuritis
How does arsenic poisoning work - what’s the mechanism?
inhibits pyruvate dehydrogenase, blocks oxidative phosphorylation so causes acidosis
Treatment for arsenic poisoning?
Dimercaprol (BAL) or succimer (chelators) + symptomatic support. Takes 1-3 weeks to recover. Use glucocorticoids for dermatitis
What are labs/findings for acute toxicity?
Hypotension, acidosis so increased respirations
what are labs/findings for chronic toxicity?
Glove and stocking distribution of burning sensation. Bilateral arm and leg weakness, Hyperpigmentation and thickening of skin on palms and soles, Aldrich-Mees lines on nails.
