Approach to Cardiac Arrest Flashcards
What are the three things that must be done when seeing a patient with a suspected cardiac arrest?
- BLS survey
- Fingerstick glucose
- ACLS secondary survey
Do you take out or leave in dentures when bagging a patient?
Leave in–forms a better seal
How long should the carotid pulse be checked for in the ACLS algorithm?
No less than 5 seconds, but no more than 10
What are the components of the NAVEL mnemonic for the drugs that are safe for endotracheal administration?
Naloxone Atropine Vasopressin Epi Lidocaine
What are the 5 Hs?
Hypoxia Hypovolemia Hypothermia H+ (acidosis) Hyper/hypokalemia
What are the 5 Ts?
Tension pneumo Tamponade Toxins Thrombosis (lungs) Thrombosis (heart)
What is the defibrillation joules needed for biphasic and monophasic defibrillators respectively?
Mono = 360j Bi = 200j
When should a pulse check occur after a defibrillation?
2 minutes or five cycles
How often are pulse checks done?
Every 5 minutes
How often is epi given in ACLS?
q3-5 minutes (or after every other 2 min pulse check)
What is the amount of epi given in ACLS?
1 mg
What is the amount of atropine given in ACLS? How many total doses?
1 mg q3-5 minutes
3 total doses
After how long of resuscitation efforts is it appropriate to consider cessation?
After 20 minutes–likely dead and/or brain dead
What are the 6 inclusion criteria for therapeutic hypothermia post arrest?
- Pt resuscitated after out-of-hospital arrest with Vt/VF as initial rhythm
- Resuscitated started within 15 minutes by EMS
- No more than 60 minutes after ROSC
- Adult age
- Tubed and mechanically vent
Can a pregnant woman be treated with therapeutic hypothermia after arrest?
No
