Define appendicitis
A
Inflammation of the appendix. Most common medical emergency

Pathology of appendicitis
A
- Obstruction of the lumen from faecolith (hard mass of faeces) causes gut flora to invade appendix wall
- Inflammation and infection leads to oedema, ischaemic necrosis and perforation
- Liable to perforation, letting infected matter and faeces into the peritoneal cavity -> peritonitis
- Alternatively could form an appendix absces

Symptoms of appendicitis
A
- Periumbilical pain that moves to the RIF
- Vomiting
- Diarrhoea
- Loss of appetite

Signs of appendicitis
A
- Fever
- Tachycardia
- Rebound and percussion tenderness in RIF
- Rovsing’s sign - more pain in RIF than LIF when LIF is pressed
- Psoas sign - pain on extending hip (if retrocaecal appendicitis)

Investigations for appendicitis
A
- Bloods - CRP elevated, neutrophil leucocytosis
- US can be useful, appendix not always visualised
- CT - gold standard - high diagnostic accuracy

Management of appendicitis
A
- Appendicectomy - open or laparoscopic
2. IV antibiotics - piperacillin/tazobactam

Complications of appendicitis
A
- Peritonitis
