Define alcoholic liver disease
Chronic liver disease as a result of excessive alcohol consumption, including fatty liver, alcoholic hepatitis, and chronic hepatitis with fibrosis or cirrhosis
Pathology of alcoholic liver disease
Fatty liver -> alcoholic hepatitis -> cirrhosis
- Fatty liver - alcohol causes reduced NAD - less oxidation of fat so it builds up so steatosis. Hepatocyte injury due to oxidative stress
- Alcoholic hepatitis - bloated hepatocytes contain Mallory bodies and neutrophils surround them. Fibrosis and foamy degeneration of hepatocytes.
- Cirrhosis - irreversible damage - destruction of liver architecture and fibrosis
Pathology of alcoholic liver disease

Early signs and symptoms of alcoholic liver disease (6)
- Abdominal pain
- Nausea
- Diarrhoea
- Malaise
- Fatigue
- Loss of appetite
Advanced signs and symptoms of alcoholic liver disease (think what liver functions are affected)
- Jaundice
- Oedema
- Ascites
- Fever
- Pruritis
- Hair loss
- Clubbing
- Palmar erythema
- Significant weight loss
- Muscle wasting
- Dark stool and haematemesis
- Confusion and memory problems
- Insomnia
- Easy bleeding/bruising
CAGE screening
- Have you ever felt you need to CUT DOWN?
- Have you ever felt ANNOYED at someone telling you to reduce drinking?
- Have you ever felt GUILTY about drinking?
- Have you ever had an ‘EYE-OPENER’ (drink in the morning to prevent hangover + steady nerves)
RF for alcoholic liver disease
- Obesity
- Women
- Pre-existing liver condition (e.g hep C)
- Genetics (alcohol dependance runs in the family)
Management of alcoholic liver disease
- STOP DRINKING
- Withdrawal symptoms - benzodiazepine
- Prevent relapse - acamprosate and dilsufiram (+CBT)
- Diet - low salt, high carb + protein, more small meals
- Corticosteroids - for liver inflammation
- Thiamine - treat malnutrition
- Liver transplant
Management of alcoholic liver disease
- STOP DRINKING
- Withdrawal symptoms - benzodiazepine
- Prevent relapse - acamprosate and dilsufiram (+CBT)
- Diet - low salt, high carb + protein, more small meals
Conditions for granting liver transplant
- Develop liver failure despite not drinking
- Fit for surgery
- Commit to not drinking for the rest of their life
Complications of alcoholic liver disease
- Portal vein hypertension and varices -> split varices can lead to massive bleed or anaemia
- Ascites -> peritonitis
- Hepatic encephalopathy
- Infection
- Liver cancer
