Define alcoholic liver disease

A

Chronic liver disease as a result of excessive alcohol consumption, including fatty liver, alcoholic hepatitis, and chronic hepatitis with fibrosis or cirrhosis

Pathology of alcoholic liver disease

A

Fatty liver -> alcoholic hepatitis -> cirrhosis

  1. Fatty liver - alcohol causes reduced NAD - less oxidation of fat so it builds up so steatosis. Hepatocyte injury due to oxidative stress
  2. Alcoholic hepatitis - bloated hepatocytes contain Mallory bodies and neutrophils surround them. Fibrosis and foamy degeneration of hepatocytes.
  3. Cirrhosis - irreversible damage - destruction of liver architecture and fibrosis

Management of alcoholic liver disease

A
  1. STOP DRINKING
  2. Withdrawal symptoms - benzodiazepine
  3. Prevent relapse - acamprosate and dilsufiram (+CBT)
  4. Diet - low salt, high carb + protein, more small meals
  5. Corticosteroids - for liver inflammation
  6. Thiamine - treat malnutrition
  7. Liver transplant
