Why is appendicitis a medical emergency?

  • It is inflammation of the appendix that can lead to ischaemia, oedema, necrosis and perforation—-> widespread peritonitis

‘lumen obstruction due to lymphoid hyperplasia, faecolith or filarial worms

A

A

1) Gradual onset of poorly localised pain (umbilical region)

2) pain moves to right iliac fossa becomes sharper, constant and well localised.

A

GI:

  • Mesenteric Adenitis
  • Meckel’s diverticulitis
  • Crohn’s
  • Carcinoma of caecum
  • perforated ulcer

Gynae:

  • Ruptured ovarian follicle
  • acute salpingitis
  • ectopic pregnancy
  • torsion of ovarian cyst

Urinary:

  • Pyelonephritis
  • Ureteric colic
  • UTI
  • Right sided testicular torsion

  • Urine analysis (microscopic haematuria and proteinuria)

  • FBC (raised neuts)
  • CRP/ ESR raised

What is the management of appendicitis?

A
  • laparoscopic appendicectomy

- prophylactic antibiotics
Metronidazole and cefuroxime IV
