APOM purpose
Routine film but best to see the dens and arches of atlas
If dens is not present
Agenesis of the dens
Fx through dens tend to go
Oblique
Smiling arch
Posterior arch
Frowning arch
Ant arch
Mach lines
Arches of atlas
Occiput - vertical radiolucency
Teeth - fx = horizontal radiolucency
Check the base of dens for a radiolucent line
Fracture - thin and jagged radiolucency with displacement
Non-union - thick radiolucency, smooth cortices
Trace out the dens making sure it is in place
Leaning or tilting dens suggests fx
Para-odontoid spaces
If unequal think Jefferson bursting fracture
Lateral masses check to confirm
Check lateral masses
Overhanging Jefferson bursting fracture Associated with RUst sign Refer for surgical stabilization Go to lat C2 body - draw staright vertical line up
Check posterior arch of atlas and spinouses of C2 and C3 for spina bifidas
Refer to orthopedist all 3 dens fx

- Thin radiolucent line above dens base (tip
- Thin radiolucent line through dens base - leaning/tilting dens
- Thin radiolucent line below dens base
