What is a 12 month period?
12 month measured backwards from date leave is taken
How is “Child” defined?
Biological, adopted, foster, step child, legal ward, child of a domestic partner, who is:
Under 18 years of age
Over 18 but incapable of self care due to mental of physical disability
How is parent defined?
Biological parent of an employee or a person who stands in place of parent
How is spouse defined?
Husband, Wife, Domestic partner
What is a serious health condition?
Illness, Injury, impairment or physical or mental health condition that involves:
- Inpatient Care
- Continuing care by health care provider
- Period of incapacity more than 3 consecutive days
How long does Pregnancy disability leave give you?
4 months or 88 work days of leave
What are some reasons for FMLA?
- Birth of child or to care for a newborn of the employee
- Placement of a child in adoption or foster care
- Care for child or spouse with a “Serious Health Condition”
- Own serious health condition
Who is eligible for FMLA?
- Been employed 12 months
2. Has worked 1250 hours with the city in 12 months
How much leave is granted to the employee?
12 work weeks, (672 hours for suppression employees
If possible how far in advance should you let the city know of FMLA?
30 days if possible
