What is an aortic aneurysm?
Dilation of >1.5x the artery’s original size
What is the normal aortic diameter?
1.2 - 2 cm
What are common sizes of aortic aneurysms?
Aortic (mainly infra-sternal)
Iliac
Femoral
Popilateal
Despite common misconception, aortic aneurysms are NOT caused by?
Atherosclerosis
What is the pathophysiology of aortic aneurysm?
Degeneration of tunica media (imbalance of breaking/building proteins) Weakening of arterial wall Dilation Increase aortic wall stress Progressive dialtion Can rupture w/o intervention
What are risk factors for aortic aneurysm?
Smoking Age Male Hypertension Hx of aneurysm
What are signs/symptoms ?
Abdominal pain radiating to the gal, iliac fossa, or femoral
Expansile abdominal mass - pulse transmitted towards flanks
What are investigations to diagnose?
USS
CT scan with contrast
What are complications of aortic aneurysm?
Rupture, thrombosis, embolism, fistulae
Pressure on other structures
How can aortic aneurysms be prevented?
Regular USS
Treat Hypertension
Who should be screened?
Men > 65
What are treatment options?
Stenting (Endovascular via femoral)
Surgery - open repair / EVAR
Rupture of an aortic aneurysm requires?
Emergency surgical repair
