Q

MOA and MOR of aciclovir, valaciclovir, penciclovir, famciclovir?

A

MOA - Nucleoside analogue

MOR

Q

MOA and MOR ganciclovir and valganciclovir?

A

MOA - nucleoside analogue

MOR
1. Mutation in the UL97 encoded CMV phosphotransferase - low level resistance to ganciclovir but NOT to foscarnet or cidofovir

  1. Mutation in UL54 encoded viral DNA polymerase - high level resistance to ganciclovir and cross resistance to foscarnet or cidofovir

Q

MOA and MOR for oseltamivir, zanamivir, peramivir?

A

MOA - selectively binds to an unoccupied area of viral neuraminidase and inhibits the enzyme, prevents spread of virus in resp tract

MOR - mutation in neuraminidase

MOR Zanamivir - mutation in either neuraminidase or haemagglutinin or both

Q

MOA and MOR amantadine and rimantadine?

A

MOA - only active vs inf A, blocks viral m2 protein ( at low conc - prevents viral uncoating, at high conc - increases lysosomal pH and inhibits virus induced membrane fusion)

MOR
- mutation of m2 protein usually occurs during therapy which can confer cross resistance to rimantadine

Q

MOA and MOR ribavirin, S.E, CI?

A

MOA - guanosine analogue interferes with nucleic acid synthesis - block mRNA production

MOR - not demonstrated

S/E - dose related anemia, marrow suppression at high doses, itch, nausea, depression, cough

C/I - pregnancy (teratogenic) so use contraception for 6 months after therapy for both men and women, condoms (present in semen)
