Class 1A: procainamide, quinidine
monitor for widening of the PR, QRS, or QT intervals. side effect - hypotension, heart failure
Class 1B: lidocaine, mexiletine hydrochloride, tocainide, hydrochloride
CNS side effects: slurred speech, confusion, drowsiness, seizures; hypotension and bradycardia.
Class 2: propanolol, esmolol hydrochloride, sotalol hydrochloride
apical heart rate! assess for SOB/wheezing, fatigue/sleep disturbances, apical heart rate for 1 MINUTE BEFORE ADMINISTRATION
side effects: bradycardia, hypotension, bronchospasm, increase in heart failure
Class 3: Amiodarone hydrochloride
CONTINUOUS EKG MONITORINGalso QT interval. photophobia - teach to wear sunscreen and sunglasses.
side effects - liver toxicity! muscle weakness, hypotension and bradycardia and AV BLOCK
Class 4: Verapamil; diltiazem hydrochloride
Monitor APICAL HEART RATE AND BP, orthostatic precautions, report signs of heart failure to health care provider
general considerations -
most side effects deal with monitoring heart stuff and worsening heart failure.
