Class I: Sodium
Class II: Beta Blockers
Class III: Potassium
Class IV: Calcium
Class I divisions
Sodium blockers.
Double quarter pounder,
lettuce tomato mayo
Fries please
IA Disopyramide, Quinidine, procainamide,
IB lidocaine, mexiletine, tocainide,
IC Flecainide, propafenone.
What is the potency of class I antiarrh?
IC>IA>IB
What are class II anti-arry?
Beta blockers
What are drugs in class III anti-arry?
Class III IS BAD
Ibutilide
Sotalol
Bretylium
Amiodarone
Dofetilide/Dronedarone
What are they drugs of class IV?
Verapamil
Diltazem
What are the drugs given for Afib?
1st cardiovert- Ibutilide
Then use AVN blockers: CCB, b-blockers, digitalis, or amiodarone
+
Thromoembolic events- antiplatelets and anticoagulants
How do you maintain a NSR with chronic therapy for AFib?
Dofetilide
- but check for ventricular tach- QT
Class I agents are being pursued becuase?
Increased mortality
What is the best medication to give patients post MI?
B blockers
When do you use Adenosine?
Adenosine slows conduction through the AVN.
Good for ventricular porblems.
When do you use digoxin?
It increases vagal tone, parasympathetics. Good for Afib in pt with HF.
When do you use atropine?
To treat bradycardia
What is the most effective AAD for atrial and ventricular arrhythmias?
Amio- but its TOXIC.
Many AADs have a _ QT interval which predisposes the a _ _ _ called _ _ _
Increased QT
Polymorphic ventricular tachycardia
Torsades de pointes
