What muscles do you inject in the glabellar region (between the eyes) to treat frown lines? And why?
The procerus and corrugator supercilii muscles.
- The procerus muscle contracts to cause horizontal lines.
- The corregator supercilii contract to cause vertical lines.
Anti-wrinkle injections in both muscles are required to inhibit frown lines here.
How do you inject into the procerus and corrugator supercilii muscles (depth, angle etc.)?
PROCERUS
Pinch the procerus muscle before injecting and insert the full needle due to the depth of the muscle; inject at 90 degree angle.
CORRUGATOR SUPERCILLI
You push the inner corner of the eyebrow up and inject the head of the corrugator supercilii muscle all the way at a 90 degree angle. This is because the head of the muscle lies deeper down.
You then push the inner corner of the eyebrow and inject the tail of the corrugator supercilii muscle only using 1/3 of the needle and at a 30-45 degree angle. This is because the tail of the muscle is more superficial.
FEEL AND LOOK FOR THE CORRUGATOR AS IT SITS DIFFERENTLY ON INDIVIDUALS.
How do anti-wrinkle injections work? (client friendly version)
- The product blocks the transmission of a chemical, acetylcholine, from the nerves to the muscle.
- Acetylcholine is a neuro-transmitter that sends a message telling the muscle to contract.
- When this message is blocked or reduced, the muscle doesn’t react and therefore relaxes (rather than contracts).
- The area is therefore less wrinkled.
How far above the orbital rim must injections be placed to de-risk issues with a dropped brow (aka Ptosis)?
No less than 1cm above the orbital rim (one finger).
If ptosis occurs, when will it show and how long will it reasonably last for?
Upper lid ptosis can be seen up to 2 weeks after the injection and may last between 2 -4 weeks.
What does contraction of the frontalis muscles do?
The paired muscles raise the eyebrows and upper eyelids. This produces traverse creases in the forehead.
Why is there no benefit to injecting in the centre of the upper forehead, near the hair line?
The frontalis muscles do not meet therefore there is no muscle there.
What layer of tissue should anti-wrinkle injections be placed in the frontalis muscles?
Supercifial subcontaneous tissue - inject at a 45 degree angle.
After treating the glabellar region, what is the ‘no go’ zone on the forehead for further injections?
The lower half of the forehead is the stopper line. Assess for each client before administering frontalis injections.
What safety gap must you have before treating crows feet?
Inject at least 1cm away from the orbital rim (one finger width).
How deep do you inject anti-wrinkle treatment for crows feet?
Superficially at a 30 degree angle.
Why must you be careful not to administer botox too low around the eye?
To avoid injecting the zygomaticus major muscle as this will impact the eye and mouth.
What direction do we inject the anti-wrinkle treatment for crows feet?
Away from the eye!
What is the skin thickness of different areas of the face?
Forehead: 1mm
Glabella: 0.7mm
Immediate under-eye area: 0.4mm
Under eye/outside of socket: 1mm
Cheeks: 1.1mm
Nose tip: 1.2mm
Cupids bow: 0.8mm
Upper chin: 0.8mm
What different angles should the needle be injected in the superficial, mid and deep parts of the skin?
Superficial: 10 degrees
Mid: 30 to 40 degrees
Deep: 45 to 90 degrees
