Anti Parkinsonism Agents Flashcards
Drug Names for Dopaminergics
Antiparkinsonism Agents
Amatadine levodopa carbidopa-levodopa rasagiline ropinirole
Action for Dopaminergics
Antiparkinsonism Agents
- Increasing the levels of dopamine in substantianigra.
- Directly stimulating the the dopamine receptors in that area
- Helping to restore the balance between the inhibitory and stimulating neurons.
indication for Dopaminergics
Antiparkinsonism Agents
relief of the s/sx of idiopathic Parkinson’s disease
Adverse effects for Dopaminergics
Antiparkinsonism Agents
anxiety nervousness HA blurred vision arrhythmias
contraindication for Dopaminergics
Antiparkinsonism Agents
known allergy
angle closure glaucoma
GI obstruction
P/L
Cautions for Dopaminergics
Antiparkinsonism Agents
CV disease bronchial asthma Hx of peptic ulcer urinary tract obstruction psychiatric disorders
drug/drug for Dopaminergics
Antiparkinsonism Agents
MAOIs vitamin B6 tyramine containing foods st johns wort meperidine and other analgesics
Levodopa
mainstay of tx for parkinsonism precursor of dopamine that crosses the blood-brain barrier where it is converted to dopamine
almost always given in combo w/ carbidopa as fixed combo drug
carbidopa decreases the amount of levodopa needed to reach a therapeutic level in the brain
dosage can be decreased, decreasing ADE
Nursing Diagnosis for Dopaminergics
Antiparkinsonism Agents
disturbed though process r/t CNS
risk for urinary retention r/t dopaminergic affects
constipation r/t dopaminergic effects
risk for injury r/t CNS and incidence of orthostatic HTN
deficient knowledge
Assessment for Dopaminergics
Antiparkinsonism Agents
allergy, GI depression or obstruction, urinary resistance or obstruction, BPH or glaucoma, cardiac arrhythmias, HTN or respiratory disease, P/L, renal and hepatic dysfx
assess skin for lesions, hx of melanoma or CNS
gait tremos spasticity, LS, VS, BS, urine output
Implementation for Dopaminergics
Antiparkinsonism Agents
arrange to decrease dose if therapy is interrupted
evaluate disease process and s/sx periodically, record for reference
give with meals
monitor bowel fx, bowel program if constipation is severe
void before taking drug for urinary retention
monitor urinary output, palpate bladder, check for residual urine
safety precautions for CNS and vision changes
monitor hepatic, renal and hematological test periodically
evaluation for Dopaminergics
Antiparkinsonism Agents
response to drug:
improvement in s/sx of Parkinson’s disease
monitor ADE:
CNS changes, urinary retention, GI depression, tachycardia, increased sweating, flushing
Drug names for Anticholinergic Agents
parkinsons
Benztropine
Diphenhydramine
indication for Anticholinergic Agents
parkinsons
synthetic drug w/ greater affinity for receptor sites in the CNS than peripheral nervous systems.
