What is an ankle fracture?
Fracture of medial, lateral or posterior malleolus
How are ankle fractures classified?
Isolated medial malleolar fracture
Isolated lateral malleolar fracture
Bimalleolar fractures
Trimalleolar fractures
What is the Weber classification?
Classification of lateral malleolus fractures
Type A = below syndesmosis
Type B = at level of syndesmosis
Type C = above syndesmosis
Clinical features of ankle fractures
Ankle pain
Unable to weight bear
+/- deformity
Check neurovascular status
What are the Ottawa ankle rules?
X-rays only required if
- bone tenderness at posterior edge of lateral malleolus OR
- bone tenderness at posterior edge of medial malleolus OR
- an inability to weight bear both immediately and in ED
Investigations for suspected for ankle fracture
X-ray
- lateral and Mortise view
Conservative management of ankle fractures
Below knee back slab
- neurovascular exam
- post-manipulation x-ray
Surgical management of ankle fractures
ORIF
Indications
- Bimalleolar/trimalleolar fracture
- Weber C or B with talar shift
- open fracture
