Determinants of Myocardial Oxygen Demand
Wall stress, heart rate, contractility
Nitrate Mechanism
Acts as NO. In low dose venodilation and high dose vasodilation, decreases preload, decrease wall tension, Increased O2 delivery
Nitrate Administration
Sublingually to avoid first pass effect. Can be given acute or chronic
Nitrate Side Effects
headache, hypotension, reflex tach, NO BENEFIT FOR MORTALITY
B-blocker mechanism
block B-Adrenergic receptors decreasing sympathetic tone. Overall less O2 consumption and some perfusion, SHOW DECREASE IN MORTALITY increase
B-blocker side effects
excessive bradycardia, bronchoconstriction, exacerbate diabetic control, use B1 selective for asthmatics
Ca+2 blocker mechanism
coronary dilation, decreased afterload, SOME have cardiac depression
Dihydropyridines
Ca+2 blockers only affecting coronary dilation
Diltiazem, Verapamil
Ca+2 blockers with coronary dilation and cardiac depression
Ca+2 blocker side effects
bradyarrhythmias, hypotension, ankle edema
Ranolazine
blocks late phase inward Na+
