ANESTHETICS INDICATION
pain-controlled analgesia (PCA)
ANESTHETICS ROA
General: IV, inhalation
Regional: *intrathecal, *epidural, infiltration anesthesia, peripheral nerve block, IV, regional block Local: injection, topical
ANESTHETICS MOA
reversibly bind a receptor site within the pore of the Na+channels in nerves -> block ion movement through the pore -> blocks action potential for nerve conduction, especially at small myelinated axons that carry nociceptive input
ANESTHETICS AE
rare but possible, especially if drug enters systemic circulation: CNS stimulations (tremors, confusion, seizures) progressing to CNS depression.
CV: arrhythmia, bradycardia, hypotension, cardiac arrest. Respiratory depression
ANESTHETICS PT SPECIFIC CONSIDERATIONS
Neuromuscular weakness (prolonged drowsiness, potential fall risk, mobilizing patient may positively reduce muscle relaxant drugs). Impaired airway clearance (depress mucociliary clearance, encourage patient to cough and deep breathe -spirometer, expel anesthetic gases and pooled secretions, postural drainage)
ANESTHETICS PK/PD CONSIDERATIONS
Can suppress T- B-cell function up to 1 month post-op, older adults - pulm complications increase, reduced cough reflex -> increased risk for pneumonia and post-op atelectasis
