What is the pain pathway ?
Transduction -> transmission-> modulation-> perception
What is the purpose of analgesics
Balanced anesthesia and pain management
A drug with good affinity for the receptor and the ability to produce intrinsic effect
Agonist
A drug with good affinity but little or no intrinsic effect
Antagonist
This opioid is most often given in patches or CRI because it is short acting. Pure MU agonist. Last 30min-2hours.
Fentanyl
Short acting mixed agonist/antagonist
Butorphanol
Partial Mu agonist and kappa antagonist. Last 6-8hrs, given SQ,IM, IV, transmucosal. Not as strong.
Least likely to produce negative effects
Buprenorphine
Partial agonist/antagonist strong kappa agonist. Short acting analgesic 30-60min. Mostly used for sedation. Can be given IM or IV
