Amniotic Fluid Flashcards
pH of Amniotic Fluid.
7.2-7.4
Nitrazine Paper test.
Useful in diff btw vaginal and amniotic fluid in cases of PROM.
Amniotic Fluid turns blue to indicate alkaline pH.
Amniotic fluid is max at
32-34 wks.
1Ltr.
Major Contributor of amniotic fluid at
1) 1st Trimester
2) 12-20 wks.
3) > 20 wks
1) Maternal Plasma
2) Fetal skin - transudate
3) Fetal urine.
Major contributor of amniotic fluid
Fetal urine
When does fetal urine production begin
12 wks
Amniotic fluid colour at term
Straw coloured
Green amniotic fluid
meconium
seen in Fetal distress , Breech or transverse lie, Listeria infection.
Golden amniotic fluid
Rh Incompatablility
Tobacco juice colour
Intrauterine death.
Saffron / Yellowish green amniotic fluid
Post dated pregnancy
Dark Colour amniotic fluid
Abruptio placenta
does amniotic fluid have nutritive function
no
Composition of amniotic fluid
99% water.
Best investigation to measure amniotic fluid
AFI - amniotic fluid index.
Normal - 5 - 24cms
Oligohydramnios . Dx
AFI < 5ms.
Single largest vertical pocket- <2cms.
Volume <200ml
Polyhydramnios- Dx
AFI >25cms
Single largest vertical pocket >8 cms.
Volume >2 lts
MCC of mild Oligo/Polyhydramnios
Idiopathic
MCC of Severe OligoHydramnios
Malformation - MC - renal anomaly
MCC of Severe Polyhydramnios
GI malformations > NTD
Causes of Oligohydramnios
DILMPPAR.
Idiopathic, Renal anomalies, Drugs like indomethacin and ACE’s, IUGR, Leakage following amniocentesis, Maternal PIH, PROM, post term pregnancy, Amnion nodosum, Chromosomal Abnormalities, Renal Anomalies
Causes of Polyhydramnios
Multifetal pregnancy, Maternal DM, high Fetal CO, Swallowing defects. Trisomy 18 and 21, NTD, Chorangioma of placenta and Torch infxn
Effects of Oligohydramnios on Pregnancy
1) Less space for fetus to grow - MC Lung Hypoplasia
2) Amniotic band Seq.
3) Meconium Aspiration in late term Oligohydramnios. leading to fetal distress.
Amniotic band seq.
Happens in oligohydramnios.
Tightening or wrapping of membrane around fetus leading to devascularization.
MC- Limb amputation>Craniofacial defect
Effects of Polyhydramnios in pregnancy
PROM/ Preterm labour - Stretching of membrane.
Resp Distress to mother.
Abruptio Placenta
Cord Prolapse
Malpresentations.
Amniotic fluid embolism
PPH due to loss of tone due to overstretching
Effects of Oligohydramnios in late term pregnancy
1) Meconium aspiration
2) CTEV. Cong Talipes Equinovarus.
TOP of Preg in idiopathic Oligohyd.
37-38 wks + 6 d
Mx of Oligohyd.
Amnio Infusion of NS
Mx of Polyhyd.
Serial amniocentesis in cases of severe maternal resp distress.
Indomethacin before 32wks.
