streptomycin
kanamycin
gentamycin
amikacin
neomycin
2
Q
mech of action
A
bactericidal by acting on intracellular protein synthesis
NO NOT ACT OF CELL WALL
3
Q
use
A
aerobic gram neg. bacteria
gentamycin broader spectrum, toxic levels if > 9mcg/ml
neomycin adjunct therapy to hepatic coma (lowers NH3 with given PO)
4
Q
met/elim
A
extensive renal excretion
T1/2 - 2-3 hr
5
Q
side effects
A
ototoxicity - dose dependent, irreversable
nephrotoxicity (tubular necrosis, reversable
skeletal muscle weakness (decreased release of ACh and decreased sensitivity post synaptically)
prolongs NMB
6
Q
anesthesia considerations
A
1st sign of nephro-toxicity is inability to concentrate urine
caution in myasthenia gravis
