what is primary amenorrhoea?
A
failure to start menses by age of 16

what is secondary amenorrhoea?
A
cessation of established, regular menstruation for 6 months or longer

what are the causes of primary amenorrhoea?
A
- consitutional delay
- late bloomer, has 2˚ sexual characteristics
- Turner’s syndrome
- XO chromosome
- testicular feminisation syndrome
- XY genotype, no internal female organs
- congenital adrenal hyperplasia
- Kallmann syndrome
- failure to secrete GNRH
- congenital malformations of the genital tract
- imperforate hymen
- mullerian agenesis

what are the causes of secondary amenorrhoea (after excluding pregnancy)?
A
- menopause
- contraception
- lactational amenorrhoea
- hypothalamic amenorrhoea (e.g. stress, excessive exercise)
- PCOS
- hyperprolactinaemia
- premature ovarian failure
- thyrotoxicosis*
- Sheehan’s syndrome
- Asherman’s syndrome (intrauterine adhesions)
*hypothyroidism may also cause amenorrhoea

what are the initial ix done for amenorrhoea?
A

urinary / serum bHCG
- exclude pregnancy

gonadotrophins
- low levels = hypothalamic cause
- high levels = ovarian problem e.g. premature ovarian failure
- prolactin
-
androgen levels
- raised levels in PCOS
- oestradiol
- TFTs
