Age >50 Fam hx Smoking HTN CVS disease Obesity
Pathophys of Wet and Dry
Dry: Geographic atrophy of macular cone cells
Wet: Choroidal vascularisation of sub-retinal space causing exudate formation.
Symptoms/signs
Loss of visual acuity
Blurring of vision
Scotoma (alteration of visual field)
Acute onset suggests wet, gradual is dry
Ophthalmoscope:
Macular haemorrhage, exudate formation, pigmentation.
Amsler grid. Drusen.
Investigation
Referral to opthalmology
Investigation will include Optical Commuted Tomography
Slip lamp microscopy
Colour fundus photograph.
Management for dry and wet
Dry: Visual rehabilitation
smoking cessation
Explain Charles Bonnet hallucinations
Balanced diet and decreasing BMI
Wet: Intravitreal anti-VEG F (bevacizumab)
