ALS Flashcards
What are the criteria to indicate CPR should be commenced?
Not conscious, not responsive, not breathing normally. Consider listening for pulse centrally.
When do you give adrenaline in SHOCKABLE rhythms?
After the SECOND cycle/ shock, then every second cycle
When do you give adrenaline in NON-shockable rhythms?
after the FIRST cycle
When do you give amiodarone in ALS?
Only for shockable rhythms, after the THIRD cycle/ shock
What dose of ADRENALINE do you give in resuscitation?
1mg (1 in 10,000, i.e. 1ml)
What dose of AMIODARONE do you give in resuscitation?
300mg
What are the 4 ‘H’s and Ts in ALS?
Hypoxia
Hypovolaemia
Hyper/ HypoKalaemia, metabolic disturbances
Hypo/Hyperthermia
Tension Pneumothorax
Tamponade
Thrombosis
Toxins
What oxygen saturation range should you aim for in resuscitation?
15L high flow initially, then aim for 94-98% once more stable.
What oxygen saturation should you aim for in a known CO2 retainer/
89-92%
What imediate treatment for ACS should be considered?
Morphine
Oxygen
Aspirin (300mg dissolvable)
Nitroglycerin
If a patients BSL is <4.0 in a resuscitation scenario, what should you give?
50ml of 10% glucose. Keep giving every minute if needed.
What should you do during initially during ALS, prior to starting compression.
D - Ensure no immediate danger.
R - Assess for response, verbal then shake
S - send for help
A
- Open airway using head tilt + chin lift
- If risk of cervical spine injury then just use jaw thrust,
B
- assess for breathing (cheek next to airway + looking at chest), feel carotid at same time
C
- start compressions
What rate should you perform chest compressions at?
100-120 BPM
What does COACHED in ALS stand for?
Compression continues Oxygen Away All Clear Charging Hands Off Evaluating Discharging/ Dumping
When are stacked shocks acceptable:
If patient monitored and witnessed cardiac arrest
Well oxygenated prior
Shock can be delivered with in 20 seconds (needs manual defibrillator)
