Alprostadil, also known as prostaglandin E1, stimulates adenyl cyclase, increasing cAMP levels. cAMP activates protein kinase A, leading to smooth muscle relaxation in the arterial blood vessels and sinusoidal tissues of the corpora. This results in improved blood flow and filling of the corpora, aiding in erectile function.
Why is alprostadil preferred for intracavernosal therapy in treating erectile dysfunction?
Alprostadil is preferred for intracavernosal therapy because it has been FDA approved for erectile dysfunction, does not require extemporaneous compounding, and has a low potential for causing prolonged erections and priapism. Its efficacy is attributed to excellent bioavailability when injected directly into the corpora cavernosum.
