Allergic Disorder Flashcards
What is hypersesntivity pneumonitis?
inflammation of the alveoli and distal bronchioles caused by an immune response to inhaled allergen
Why is it imporant to ask about occupation?
Occupational exposure to organic dust is the key epidemiological factor - most commonly including Actinomycetes bacteria, animal proteins, or reactive chemicals.
What is the patho of HP?
also known as extrinsic allergic alveolitis, is the result of non-IgE mediated immunological inflammation
What is the presentation of allergic disorder?
- Exposure to avian protein antigen, especially excrement
- Mould in work environment
- Occupation exposure to chemicals
- Dyspnoea
- Non-productive cough
- Productive cough
- Fever/chills
What Ix is done in allergic disorder?
- ESR: high
- Albumin: low
What is DDx for allergic disorder?
- Viral pneumonia
2. Sarcoidosis
What is Mx of allergic disorder?
1st line: avoidance of antigen
Corticosteroid taper
Long-term low-dose corticosteroid therapy
What are complications of allergic disorder?
- Progressive deterioration
- Hypoxaemia
- Death
What reactant is in Type 1 Hypersensitivity reactions?
IgE
What is the patho in Type 1 Hypersensitivity reactions?
- Mast-cell activation releases histamines and other mediators
- Minuets
What is an example of Type 1 hypersensitivity reaction?
anaphylaxis
What reactant is in Type 2 hypersensitivity reaction?
IgG
What is the patho of Type 2 hypersensitivity reaction?
antigens embedded in host cells cause complement activation and destruction by MAC
What is an example of Type 2 hypersensitivity reaction?
Acute transfusion reaction
What is reactant is in Type 3 hypersensitivity reaction?
IgG
