This is tough one – twister. So, one way to resolve is to flip the sentence and check which one makes sense.
A: According to public health officials, in 1998 Massachusetts became the first state in which more women over the age of thirty delivered babies than under age of thirty.[Hold it]
B: According to public health officials, in 1998 Massachusetts became the first state in which more women over the age of thirty delivered babies than born under age of thirty.[This makes invalid – eliminate it]
C: According to public health officials, in 1998 Massachusetts became the first state in which more women over the age of thirty delivered babies than they were under age of thirty.[Same women delivered over age thirty and under thirty – changes the meaning of the sentence – eliminate it]
D: According to public health officials, in 1998 Massachusetts became the first state in which more women over the age of thirty delivered babies than there had been under age of thirty.[Implies women were delivering babies – eliminate it]
E: According to public health officials, in 1998 Massachusetts became the first state in which more women over the age of thirty delivered babies than had been under age of thirty.[Does not make sense – eliminate it]

remember, “each” is singular.
“Instead of” could only put nouns in parallel,
but “rather than” can put nouns or verbs or entire actions in parallel. In general, the GMAT seems to avoid the situations in which a correct use of “instead of” would be allowed, and seems to use “instead of” only as an incorrect choice for “rather than.”
CORRECT: She simply bought a condo in Boston, rather than pay for a hotel room for several months.
In that sentence, the verbs “bought” and “pay” are in parallel. Notice that since the latter action is hypothetical, it is in the subjunctive. If this were the correct choice in a Sentence Correction question, a typical incorrect choice would be:
“she simply bought a condo in Boston, instead of paying for a hotel room for several months”
instead of => NOUN.

D is wrong because When ___ and which/when/what ___. Need to be “introduced by an interrogative pronoun.
Trick, when I see “and” I can fill in the “implicit” word present. In this case it became obvious when I said internally: “when ___ and WHEN ____”.. Was a great queue to look for missing which/what/when, etc.
The correct construction is substitutes x for y, not substitutes x in place of y. The construction following paychecks is wordy and awkward.
Why is D incorrect?
“Although wolves is a correct subject for numbering, a decline should be to a number, not to an estimate.”
Correct Answer: E. Why? Because wolves is a proper subject for numbering and agrees with the later them. The decline is correctly said to be to a number, an estimated 200,000.
