What is Wernicke’s encephalopathy?
Neuropsychiatric disorder caused by thiamine deficiency which is most commonly seen in alcoholics
Other causes of Wernicke’s encephalopathy
Persistent vomiting, stomach cancer, dietary deficiency
Classic triad presentation of Wernicke’s
ophthalmoplegia/nystagmus, ataxia and confusion
Clinical presentation of Wernicke’s
nystagmus (the most common ocular sign)
ophthalmoplegia
ataxia
confusion, altered GCS
peripheral sensory neuropathy
Investigations of Wernicke’s
decreased red cell transketolase
MRI
Treatment of Wernicke’s
urgent replacement of thiamine
What is Wernicke- Korsakoff syndrome?
characterised by the addition of antero- and retrograde amnesia and confabulation
What is Korsakoff syndrome?
marked memory disorder often seen in alcoholics
thiamine deficiency causes damage and haemorrhage to the mammillary bodies of the hypothalamus and the medial thalamus
