Tracheostomy
surgically created stoma in anterior wall of trachea
Indications for a Tracheostomy
a. establish patent airway
b. bypass upper airway obstruction
c. facilitate removal of secretions
d. permit long term ventilation
e. assist with weaning from mechanical ventilation
Types of Tracheostomy
a. Single versus double cannula
b. Fome-cuf
c. Fenestrated tracheostomy tube
d. Speaking tracheostomy valves
Prior to insertion
a. Percutaneous insertion or surgical placement
b. Verify consent
c. Check coagulation status
d. Baseline assessment vital signs, O2 saturation
e. Confirm IV access and prepare medications
f. Gather equipment
After insertion
a. Immediately after trach tube placement, inflate cuff
b. Confirm placement (auscultate breath sounds, end-tidal CO2, pass suction catheter thru tube
c. Secure tracheostomy tube
