Sx of AF?
Asymptomatic but also
- chest pain/ palpitations
- SOB
- Fainting
Sign of AF?
- Irregularly irregular pulse
- signs of LVF
4 types of AF?
Acute (<48hrs)
Paroxysmal ( <7 days, may recur)
Persistant (>7d)
Permanent (> 1yr)
Acute AF mx?
Emergency cardioversion
Control ventricular rate
Start LMWH
Control of ventricular rate meds 1st line?
diltiazem or verapamil or metoprolol
Control of ventricular rate meds 2nd line?
digoxin or amiodarone
Paroxysmal AF mx?
- Anticoagulate (chadsvasc)
- PNR: flecainide, propafenone
Prevention of AF
B blocker or
Sotalol or
Amiodarone
Antiarrhythmics for persistent AF
- Not needed if successfully treated precipitant
- 1st line: beta blockers
- 2nd line: amiodarone
Rate for rate control aim for persistent AF?
<90 bpm at rest
Rate control for persistent AF?
1st: b-blocker or CCB (not both)
2nd: add digoxin
3rd: consider amiodarone

Rate control

CHADSVAS score use?
