Adrenocortical Agents Flashcards

1
Q

What is the adrenal cortex?

A

the outer layer of the adrenal gland

produces glucocorticoids and mineralocorticoids in response to adrenocorticotropic hormone (ACTH) stimulation

also responds to sympathetic stimulation (EPI)

Q

What is the adrenal medulla?

A

the inner layer of the adrenal gland

a sympathetic ganglion, it releases norepinephrine and epinephrine into circulation in response to sympathetic stimulation

Q

What are corticosteroids?

A

steroid hormones produced by the adrenal cortex

include androgens, glucocorticoids, and mineralocorticoids

Q

What is the diurnal rhythm?

A

response of the hypothalamus and then the pituitary and adrenals to wakefulness and sleeping

normally, the hypothalamus begins secretion of corticotropin-releasing factor (CRF) in the evening, peaking at about midnight

adrenocortical peak response is between 6 and 9 am

levels fall during the day until evening, when the low level is picked up by the hypothalamus and CRF secretion begins again

Q

What are glucocorticoids and what do they do?

A

steroid hormones released from the adrenal cortex

they increase blood glucose levels, fat deposits, and protein breakdown for energy

Q

What are mineralocorticoids and what do they cause?

A

steroid hormones released by the adrenal cortex

they cause sodium and water retention and potassium excretion

15
Q

What should we monitor in children taking adrenocortical agents and when should we halt use?

A

Monitor growth and development

Halt if severe growth retardation occurs

16
Q

What should we teach adult patients taking adrenocortical agents?

A

Take in the morning

Taper the medication; do not stop abruptly

Check OTC preparations for corticosteroids (ex cortisol)

17
Q

Should we give adrenocortical agents to pregnant or lactating women?

A

NO

the drug crosses the placenta and enters breastmilk and the baby experiences the ADEs

18
Q

What are androgens?

A

male and female sex hormones

19
Q

What is the fxn of mineralocorticoids in the body?

A

Affect electrolyte levels and maintain homeostasis

20
Q

What are the possible causes of Addison’s Disease?

A

Not enough production of ACTH

Adrenal glands are not able to respond to ACTH

Adrenal gland is damaged

Removal of the gland

Prolonged use of corticosteroid hormones

21
Q

What are the s/sx of adrenal crisis (extreme addison’s disease)?

A

Physiological exhaustion

HypOtension

Fluid shift

Shock and even death

22
Q

What is the treatment for Adrenal Crisis?

A

Massive infusion of replacement steroids

Constant monitoring and life support procedures
