15
Q

What is the short synacthen test

A

test involves giving synacthen which is a synthetic AC\th

  • healthy adrenal glands will be stimulated by this and produce cortisol and the cortisol levels should be at least double
  • less than double of baseline indicates Addisons

Q

Which drug is used to replace cortisol

A

hydrocortisone

17
Q

which drug is used to replace aldosterone (a mineralocorticoid)

A

fludrocortisone

18
Q

How does someone with addisonian crisis present

A

reduced consciousness
hypotension
hypoglycaemia, hyponatremia, hyperkalaemia
patient can be very unwell

19
Q

How can Addisons crisis be triggered

A

by infection, trauma or other acute illness in someone with established Addisons
can. present in someone on long term steroids suddenly withdrawing these steroids

20
Q

what is the management of Addisonian Crisis

A

intensive monitoring if unwell
parenteral steroids (ie IV hydrocortisone 100mg stat then 100mg every 6 hours)
IV fluid resuscitation
Correct hypoglycaemia
Careful monitoring of electrolytes and fluid balance
