what are some conventional disease-modifying antirheumatic drugs (DMARDS)
methotrexate, hydroxychloroquine, leflunomide
what is the first line drug for RA
methotrexate - effective & relatively safe, inexpensive, 3-6 weeks for effect
what is the MOA for methotrexate
folate antagonist, decreae effect of B & T lymphocytes
what is the second line drug for RA
leflunomide - effective but more hazardous and expensive
what is hydroxychloroquine
antimalarial drug, helps with RA treatment, MOA unknown, 3-6 months for effect
can you take DMARDS when pregnant
NO - also not BF or live vaccines
what are some biologic DMARDS
TNF inhibitors (Etanercept), B-lymphocyte depleting agents (Rituximab), T-cell activation inhibitors(atabacept) IL-6 antagonist (tociluzumab) and IL-1 receptor antagonis (anakinra)
what are the MOA for Etanercept
suppress inflammation by inhibiting TNF - indicated for mod/severe RA if failed to respond to methotrexate
what is the black box warning for biologic DMARDs
serious systemic infections and sepsis
what are contraindications to biologic DMARDS
pregnancy, BF, live vaccines
what are some targeted DMARDs
janus kinase inhibitor (Tofacitnib)
what is the MOA for janus kinase inhibitor
inhibits JAK, one of the enzymes that initiats cytokine signaling a part of the START pathway
