torsion of the testicular appendage which is rotation of the testicular appendix around its own axis causing ischaemia
what is the testicular appendage
the appendix testis (or hydatid of morgagni) is a vestigial remnant of the mullarian duct
symptoms of torsion of the testicular appendage
testicular pain, oedema of scrotum, blue dot sign
torsion of the testicular appendage
can mimic testicular torsion so you may have to do a doppler ultrasound to rule it out but they are completely different and torsion of the testicular appendage is self-limiting only requiring conservative management
testicular torsion is
a surgical emergency
what is testicular torsion
twisting of the testis causing torsion of the spermatic cord and occlusion of the testicular artery causing ischaemia and necrosis
symptoms of testicular torsion
sudden onset severe pain in the affected testi, nausea vomiting and redness of scrotum
investigations for testicular torsion
doppler ultrasound
management of testicular torsion
emergency surgery
other causes of acute scrotal pain
epidydymitis, orchitis, acute idiopathic scrotal oedema
epidydymitis
inflammation of the epididymis most commonly caused by an STI and usually resolves with antibiotics
orchitis
inflammation of the testes most often caused by a bacteria or virus but most commonly associated with mumps
acute idiopathic scrotal oedema
self-limiting condition which causes marked oedema of the skin and dartos fasciae but does not involve anything deeper or the testis or epididymis
