An action plan is a type of program concern with a solution or improvement of a health problem.
An action plan should be SMART

STEPS IN ACTION PLANNING
-Organize a work planning group
-Conduct SituationalAnalysis
-Prioritize Problem
-Set Objectives
-Select Activities, Strategies, & resources to meet identified objectives
-Identify the target group of interest
-Identify indicators for evaluation
-Size of the group should be small enough to guarantee dynamic discussions
-Each member has to be oriented with specific roles, duties & responsibilities
-Organize a work planning group

-Socio-cultural-economic assessment
Conduct Situational Analysis
-Set your criteria
-Use the decision matrix
Prioritize Problem
-Activities should be doable; hence S.M.A.R.T.
-Strategies should be innovative
-Anticipate available resources (manpower, equipment, budget, etc.)
Select Activities, Strategies, & resources to meet identified objectives
-The action plan should be addressed the to most vulnerable group of the identified health problem
-E.g. age groups, group of people, class, community
