action (patho)
Inhibits the synthesis of prostaglandins that may serve as mediators of pain and fever.
(prostaglandins are primarily in the CNS and they are lipid compounds involved in inflammation, pain and fever)
critical side effects (4)
- Acute generalized exanthematous pustulosis (skin reaction where skin looks like it has eruptions)
- Stevens johnson syndrome: (severe skin reaction: skin peeling, fever, painful skin)
- Toxic epidermal necrolysis (more severe that SJS, starts with flu/influenza Sx, peeling of skin: necrosis)
- Hepatotoxicity (acute liver injury with high or over doses)
why is miki taking this med?
-due to back pain from previous spinal compression fracture aches due to OSTEOPOROSIS & other associated pains
What side effects have been experienced by the client? (2)
-From ASA = increase risk of renal effects
-Possibly Pancytopenia (significant decrease in the number of all blood cells as she has symptoms of DIZZINESS (but this dizziness was alongside her symptoms of cva)
What is important to know about this client in relation to the med? (3)
-MALNOURISHED pts = have higher risk for hepatotoxicity.
Miki states she does not eat much.
-ALCOHOL: unclear about her relationship with alcohol (no, some, frequent intake?)
-COGNITIVE ABILITY: unclear about the doses she takes. unclear about the total doses in a day. unclear on the time taken in between meds. her mental health can affect this.
what are the nursing actions related to this medication (4)
-PAIN assessment before and after administration
-ask abt her ALCOHOL INTAKE = can be at risk for hepatotoxicity
-Assess for rash during therapy : SJS (steven-johnson syndrome), and TEN (Toxic epidermal necrolysis), generalized exanthematous pustulosis. & discontinue if there is a RASH.
-Make sure client is not exceeding the recommended dose, as prolonged use of acetaminophen can cause negative RENAL and HEPATIC EFFECTS
what does the client need to know about this medication (2)
-CORRECT DOES = how much miki should take at a time, the time between, and the total amount in a day. Remind her that she should be keeping track on how much she intakes.
-Educate that this medication cannot be interacted with ALCOHOL as it cause HEPATOTOXICITY .
