Definition of Accumulation and Stagnation syndrome?
- Common condition for pediatric patients
- Usually caused by excess intake of food or milk
- Food accumulates in MJ and cannot be transformed leading to stagnation
- Can lead to malnutrition if prolonged
- Related to deficiency of SP/ST

Main symptoms of Accumulation and Stagnation
- Abdominal bloating
- Constipation with smelly defecation or loose stool
- Foul smell coming from mouth
- Indigestion
- No appetite
- Undigested food in stool

Extra symptoms you might see
- Crying at night
- Irritable
- Vomiting
Things to consider with diagnosis
History of improper diet or feeding
Test stool for undigsted particles
Differentiate Accumulation and Stagnation (pg 147) from Aversion to Food (Poor Appetite) pg 130 consider how constipation fits in
Poor Appetite Patterns:
1. Summer heat (4 types)
2. Spleen Dysfunction in T &T (inconsistent defecation)
3. Stomach Yin Xu (constipation)
4. SP/ST Qi Xu (could have undigested food here, this one most similar, but tongue is pale with thin coat, slow weak pulse)
Accumulation and Stagnation patterns:
Stagnation
1. Food Stagnation
2. Spleen Deficiency with Stagnation (pale with greasy tongue coating, slippery and thready pulse)
There may be no appetite with A & S however it will be secondary to the stagnation issues. There is smelly breath or smelly stool. Main organs are Stomach and Intestines.
The Summer Heat poor appetite syndromes could have belching with foul smell but not the others. If summer heat see sweating, floating and rapid pulse, maybe fever. Main organ is spleen for poor appetite
Poor appetite is most common in summer. Why?
Children have an excess of Yang and are prone to heat. There is extra heat in the environment, this can lead to more excess. Fire also generates Earth. Mother is in excess during summer meaning son is deficient and this affects spleen function.
What is the difference between:
1. Awaken the Spleen
2. Strengthen the Spleen
3. Tonify the Spleen
- Awaken the spleen: condition where the spleen is not working at all, not metabolizing anything. It is dormant. We need herbs to restart its function. Esp. summer, shuts down, main manifestation is no appetite.
- Strengthen the Spleen: Spleen is not deficient here, it simply hasa lot to metabolize. There is no deficiency of Spleen or Yang however we need a boosting function to make sure things are metabolized.
- Tonify the Spleen: This is only used when the spleen is actually deficient. Spleen Qi and Yang are usually deficient. The spleen is not working independently.
