What are Accessory cavitated uterine malformations (ACUMs)
ACUMs are isolated caveated lesions within the lateral aspect of the myometrium inferior to attachment of the round ligament
Rare mulleiran anomaly – cause severe dysmenorrhoea & pelvic pain
How to diagnose ACUMs?
Dx USS and MRI- well defined lesions with a cental cavity containing haemorrhagic content surrounded by a myometrial mantle. Histological – functional endometrial glands
How to treat ACUMs
1) Expectant
2) Simple analgesia
3) hormonal suppression - IUS or GnRH
4) destruction of the endometrial lining by alcohol sclerotherapy (risk peritoneal leakage of instilled alcohol)
5) complete surgical excision (Open>Lap)
