What is accelerated hypertension?
A
A sudden rise in BP to over 180 or 110 but not associated with acute organ impairment
As such is known as a ‘hypertensive urgency’ not a hypertensive emergency.

How do we treat accelerated hypertension?
A
With steady lowering of BP over 24-48 hours using anti-hypertensives. (not Na-Nitroprusside)
Want to lower BP by 15-20% (~110mmHg) within an hour then set 2 hour, 6 hour and 24 hour targets
Swapping out IV treatment for Oral as you go
Can also furosemide if pulm. hypertension and CCBs etc
